Co.As.It. Community Services is an approved provider under the government’s Support at Home program, also offering Restorative Care, Assistive Technology and Home Modifications (AT-HM) and End of Life Care.

Support at Home Eligibility

The Support at Home program has 8 classifications depending on your care needs and individual circumstances.

To access the Support At Home program, you will need to have an aged care needs assessment completed by My Aged Care. Once assessed, you will receive an individual support plan to share with your provider.

If you already have another service provider, but are interested in switching to Co.As.It. Community Services, you can easily change providers.

Frequently Asked Questions

No. You do not need to reapply to continue to receive services under Support at Home, if you already have an existing Home Care Package and are wanting to switch providers.

Clients can be reassured of matching benefits under the new program

  • Home Care Package recipients will keep any HCP unspent funds under the new Support at Home program
