Assistive Technology & Home Modification Support (AT-HM)

Everyone deserves to feel safe, confident and independent in their own home and our team is committed to helping you do this.

Whether it’s a grab rail in your bathroom, a ramp to enter your home or equipment that makes your daily life easier e.g. a walker or shower chair, our allied health professionals are here to support you with achieving this.

Assistive Technology (AT)

Depending your needs, our team will work with you to recommend practical equipment that supports your independence, such as:

  • Mobility aids e.g. walkers, wheelchairs
  • Shower chairs or other everyday living aids
  • Personal alarms and smart technology to help you feel safe and connected
  • Support with other devices to help you stay socially and digitally connected

We can also support you to trial the right equipment for you or set up and show you how to use your new equipment or devices so you can be confident in continuing to live independently with the right equipment or technology.

Home Modifications (HM)

Small changes at home can make a big difference to your safety at home.

Our Occupational Therapists can help organise practical home modifications that suit your needs, such as:

  • handrails for added support
  • ramps to improve access in and out of your home
  • bathroom adjustments for safety and comfort

By focusing on the changes (small or big) that matter most to you, we can help reduce risk and support your independence so you can continue living confidently and most importantly, safely in your home.

Restorative Care Services

Our Restorative Care services are designed to help you regain confidence, rebuild strength and continue living independently after a setback, illness or decline in mobility.

We work closely with you to restore function and improve your overall wellbeing – whether it’s walking safely at home, getting back to favourite activities or simply feeling stronger and more confident.

Together, our multi-disciplinary team will work with you to support you in achieving your goals that is tailored to your needs and will get you back to yourself and living independently.

Depending on your Support Plan, services may include:
