Commonwealth Home Support Program (South)

This program is funded by the Federal Department of Health.  It gives home care options to older people who wish to stay in their own homes for as long as possible.

The goal of the Commonwealth Home Support Program (CHSP) is to enhance the independence of frail older people, by providing basic maintenance, support and care services, which are essential to their well-being.

The Commonwealth Home Support Program offers a wide range of services to assist in meeting the needs of the elderly person and his or her carer, such as domestic assistance; personal care; social group activities; respite care; social support (shopping, medical appointments, accompaniment); language assistance; allied health services such as physiotherapy, podiatry, social work, occupational therapy, dietetics, speech therapy, nursing, carer support, help to assess other health professionals and community services.

Co.As.It. has the expertise and experience to cater to the needs of CALD (culturally and linguistically diverse), elderly people.

Our services are flexible and timely, provided according to your individualised care plan. Our staff will support you in achieving your own well-being goals, to allow you to continue to live in your own home for as long as possible.

To access our services, you will be required to undertake an assessment of your needs, as well as pay a small contribution. No person will be refused service provision if they are assessed as having financial difficulties.

Eligibility

All support services are provided following an assessment organised by My Aged Care (1800 200 422).  Co.As.It. will provide assistance and guidance in organising this assessment.
