PHN Brisbane North

Co.As.It. Community Services is a proud member of the PHN Brisbane North Consortium. Australians are now living longer than ever. Around 13 percent of the population in the Brisbane North PHN region (more than 124,000 people) is aged 65 years and over and this is expected to grow as a proportion over the coming decades. 

Brisbane North PHN supports communities within Brisbane’s northern suburbs, the Moreton Bay Regional Council and parts of the Somerset Regional Council, covering around 4,100km2 of urban, regional and rural south-east Queensland, with a total population of over 960,000. Its key objectives are:

  • to increase the efficiency and effectiveness of medical services for patients; particularly those at risk of poor health outcomes;
  • to improve coordination of care to ensure patients receive the right care, in the right place, at the right time. 

Healthy@home

Healthy@home is a consortium of 19 leading organisations including Co.As.It. Community Services. It is committed to providing high quality community aged care services in Brisbane North and Caboolture to help older people maximise their independence and remain living at home.

The consortium includes community aged care service providers, government and non-government agencies, peak bodies and advocacy groups. The latest articles can be found here.

Regional Assessment Service (RAS)

The Australian Government has contracted a consortium of seven local community care providers, led by Brisbane North PHN, to operate a Regional Assessment Service (RAS).  Co.As.It. Community Services is one of the seven providers.

The RAS provides personalised assessments in the Brisbane South, Brisbane North and Caboolture regions to assess eligibility for aged care services. To arrange an aged care assessment, please call the My Aged Care gateway 1800 200 422 or go to: www.myagedcare.gov.au.
