Commonwealth Home Support Program

This program is funded by the Federal Department of Health.  It gives home care options to seniors who wish to stay in their own homes for as long as possible.

The goal of the Commonwealth Home Support Program (CHSP) is to enhance the independence of seniors by providing basic maintenance, support, and care services which are essential to their well-being.

Co.As.It.’s South Coast CHSP offers three service types, these being:

  • Domestic Assistance
  • Social Group Activities (SGA)
  • Allied Health Services

Language assistance, carer support, and help in accessing other health professionals and community services are also available to all our clients.

Social Group Activities

The program offers centre-based Social Group Activities (SGA) five days per week.  The centre provides social group activities for persons who are socially isolated and in need of companionship with peers from their own culture and language background.  The centre caters to various different languages and cultural groups.

The centre offers meals, exercises, music and dancing, craft activities, bingo and other leisure activities requested by clients.  Outings are organised on a regular basis.

Transport services are provided so that the client can access these services and a care worker is available on the bus whilst transporting clients to and from the SGA centre.

A daily fee is required as a contribution towards these subsidised services.

Allied Health Services

Following a referral from a GP, we will assist you with all Allied Health Services e.g. physiotherapy, podiatry, Occupational Therapy, Dietitian, Speech Therapy, Social Work.  An assessment and care plan will identify goals achievable and improve your quality of life and increase independence.  A small contribution will be required to access these services.

Co.As.It. has the expertise and experience to be able to cater to the needs of the CALD (culturally and linguistically diverse) elderly person.

No person will be refused services if they are assessed as having financial difficulties.

Eligibility

All support services are provided following an assessment organised by My Aged Care (1800 200 422).  Co.As.It. will provide assistance and guidance in organising this assessment.
