The aged care system can be difficult to understand. Contact Co.As.It. Community Services to help with explaining what is available to you. There are various Government subsidised programs available through Co.As.It. Community Services. There are three main services that offer support to help keep your loved ones at home. The three main services are:
[1] Commonwealth Home Support Program (CHSP)
This low care option is designed for those who just need a little extra help to stay at home. Your loved ones can access a range of services that are dependent on individual needs. These services can be offered in the home, in a clinic, or at a local community centre.
These are the types of services:
- Social Support
- Transport
- Domestic Assistance
- Hygiene and Grooming
- Allied Health Services
- Nursing Services
- Home Maintenance
There are 4 simple steps you need to take to initiate Aged Care :-
(a) Contact My Aged Care (MAC) to arrange an assessment of your needs
(b) My Aged Care (MAC) will arrange a member of the Aged Care Assessment Team (ACAT) to assess your needs
(c) Nominate Co.As.It. Community Services as your preferred provider
(d) We commence looking after your needs
