Commonwealth Home Support Program (CHSP)

Co.As.It Community Services is an approved provider of this program. The goal of the Commonwealth Home Support Program (CHSP) is to enhance your independence by providing basic maintenance, support and care services, which are essential to your well-being. This program is funded by the Federal Department of Health and Aged Care. It gives you home care options to help you stay in your own home for as long as possible.

What kind of home support services are we able to offer?

The Commonwealth Home Support Program (CHSP) offers a wide range of services to assist in meeting the needs of yourself or your family member, such as:

domestic assistance; personal care; social group activities; respite care; social support (shopping, medical appointments, accompaniment); language assistance;
allied health services such as physiotherapy, podiatry, social work, occupational therapy, dietetics, speech therapy, nursing, carer support, help to access other health professionals and community services.

Co.As.It. has the expertise and experience to cater to the needs of CALD (culturally and linguistically diverse), elderly people.

Eligibility

To access our services, you will be required to undertake an assessment of your needs, as well as pay a small contribution. No person will be refused service provision if they are assessed as having financial difficulties.

The first step is to contact My Aged Care and go through registration. If you need some assistance to do this you can contact our care services coordinator and Co.As.It. can provide assistance and guidance in organising this assessment with My Aged Care (1800 200 422).
