Community Care (Under 65’s)

If you are under 65 years of age, the Queensland Department of Communities, Child Safety and Disabilities provide funding to help you remain living in your home with support services that are suited to your lifestyle and choices.

Basic support (usually 1 to 5 hours per week) is provided to people with a disability or condition that restricts their day-to-day living.

Co.As.It.’s Community Care Program offers the following types of services:

  • Domestic assistance, such as cleaning, washing and ironing
  • Personal care, including bathing, showering, dressing, and toileting
  • Meals and other food services, including provision of meals, help with food shopping and food preparation
  • Centre-based group activities, including excursions
  • Social Support, help with attending appointments, shopping or banking
  • Nursing, including wound care
  • Respite

Eligibility

In order to access our Community Care services, you must be a person, or the carer of a person, who:

  • is under 65, or under 50 for Aboriginal and Torres Strait Islander people, and
  • has a moderate, severe or profound disability, or a condition which restricts the ability to carry out activities of daily living, and
  • is living in the community, and
  • is having difficulty performing the core activities of daily living* due to functional limitations, and
  • is at risk of losing independence without assistance from Community Care.

* Core activities of daily living are communication, self-care and mobility. Core activity tasks include dressing, bathing or showering, preparing meals, house cleaning and maintenance, and using public transport.

Cost

Our services require a minimal contribution starting from $6 per hour. Special arrangements can be made if you are assessed as being financially disadvantaged.

Contact our office and ask for the Community Care Officer who will be able to organise an assessment and commence services within a short period of time.


Carers Link Accessibility APP – Access 4000

Carers Link, in partnership with Disability Support Queensland, is excited to announce the launch of their ground-breaking new app, Access 4000.

Access 4000 is the first app in Australia to provide information on Brisbane venues and infrastructure with disability access features like wheelchair/pram ramps, disabled toilets and disabled parking.

The app will help people with disabilities or anyone with accessibility needs living in Brisbane – or travelling domestically and internationally to Brisbane – to feel more informed, confident and prepared when travelling around the city and surrounding suburbs.

Our hope is that Access 4000 will be a driver for social change by encouraging inclusion and social justice for people experiencing access barriers within our communities.
