Co.As.It. Community Services is a proud member of the PHN Brisbane North Consortium. Australians are now living longer than ever. Around 13 percent of the population in the Brisbane North PHN region (more than 124,000 people) is aged 65 years and over and this is expected to grow as a proportion over the coming decades.
Brisbane North PHN supports communities within Brisbane’s northern suburbs, the Moreton Bay Regional Council and parts of the Somerset Regional Council, covering around 4,100km2 of urban, regional and rural south-east Queensland, with a total population of over 960,000. Its key objectives are:
- to increase the efficiency and effectiveness of medical services for patients; particularly those at risk of poor health outcomes;
- to improve coordination of care to ensure patients receive the right care, in the right place, at the right time. brisbanenorthphn.org.au/page/about/
